The back of this is done up from the December 1991 WWF Magazine all scanned at 600dpi, and the front at the bottom is from the ad. Rest on the front is official. And this cover in general is a "wrestling mark" type cover, because it features wrestlers who were scheduled to wrestle but didn't due to injury and the card being changed, but this cover is based on the original card.

Survivor Series (1991) was the fifth annual Survivor Series pay-per-view professional wrestling event produced by the World Wrestling Federation (WWF). It took place on Thanksgiving Eve, November 27, 1991 at the Joe Louis Arena in Detroit, Michigan. It was the first Survivor Series to feature a singles match (or a title match).
